Create Form Task_ Wait for form task

Hi All,
Right now i am working on action center.
1.read range activity-dt1
2.for each row dt1- get the variable- create the form task- add collection
4.for each- wait for form task- add collection
This is how my workflow is . i created the workflow in orchestrator process.
in read range i have 5 data. entire cycle is working fine for one cycle.
for the second cycle i am getting an error as “Object reference not set to an instance of an object.”
Value is available i dont know why am getting this error.can anyone plz guide and give suggestion.
