Upload file via Action Center is Not Working

Have a process where user need to upload file via action center. I have developed the script and when I preview through form designer, it rendered correctly:

However, when job is actually run and task is created, it only displayed the name:

Have passed the arguement:

Kindly advice

found the issue, task needs to be assigned to user for them to be able to see option to upload files.
