Unable to display PDF in Action Center using Create Form Task

Hello Everyone,

we could not able to display PDF in Action center while creating an Action Item in Action Center using Create Form Task. Kindly help on this.

In the settings of your image container in the Form Designer, check you settings as per the below image,
Also please not that syntax in the content should be as
