Not able to create Custom Activity using code as per documentation

Hello All,

I am trying to create a custom activity with Custom UI Design,
but stuck on a line from Documentation.
Trying to do the same using code by referring this link

Where I am stuck is point 18
It talks about Another activity that was not referred to in previous section of the explanation.

Can someone please share how the folder structure looks if you have created Custom activity with custom UI.
