Upload file through Action Center Form Error: Storage provider not found

Storage bucket name is test

have passed fileUpload of storageUploads type and matched to the Field key

this was working few days ago but have sudden issue as mentioned
pls assist

  1. Check if the storage bucket is removed or renamed
  2. Check if the permissions have been revoked

  2. still have access, because pdfviewer displaying pdf from storage bucket in same task is still working properly.

not sure why when to upload file we have this error
