When i use this activity, it works seamlessly but sometimes it shows this error repeatedly.
Does anyone know why it happens.

Try using UiPath Screen OCR instead of Document OCR and if issue persists then it might be source element issue, try to provide delays as captcha is not visible by the time bot is trying to extract.

To isolate cause, can you try to use TakeScreenshot and OCR engine etc instead of GetOCRText as the following?
Please check saved image when error occurs. If the image is null, it’s matter of getting image.
If there is as expected image, it’s matter of OCR engine.
