Duplicate entry(one formatted another unformatted) in excel after PDF Form based Extraction

Hello,

Can anyone help me with below issue.
Please find attached the screenshot of duplicate entries i am getting in excel after PDF data extraction via Form based Extractor. I am getting two entries for one transaction, where one entry is formatted($ symbol removed/ ‘-’ marked as .00) another is unformatted.

Also please find attached a screenshot of my sequence.
