Process Assessment Tool - UiPath - Calculation Question

The process assessment tool i have i was told was provided by UiPath, not sure if it will let me upload and example of it here as it is my first post. The calculation that is supposed to be calculate the Benefit/Suitability score doesn’t appear to be pulling from the Benefits/Suitability criteria. I was wondering if there is a base version of the process assessment tool intake form with the default calculations included that someone could share?
