How we can label check box value in document manager ml model

Hi Folks,
I am working on AI center document understanding ml model and i am trying to lable the fields to train the model i have checkbox in the document how we can lable the checkbox value there are multiple checkbox i want to extract only enable check box value.

Let me know how i can tackle for checkbox value!

I want to fetch the value of if check box is enable or not?
does anyone know how to label the checkbox in document manager
