And in the form contents-> the DocumentName.pdf is the file to upload and MetaData.json contains metadata (filename, and other common attributes) and it is required for the service call. Can someone help to setup this request?

what happens if you try sending the files as attachment?
also i set up a test flask application i passed some parameters in the body property and i see that the application is reading it in the form property. So maybe try that
if none of this works you could use invoke vb.net code
