How to create dynamic check box in UiPath action center form task

I’m creating dynamic checkboces and ratio button and select drop-down in UiPath forn tasks but it’s not working when I tried to replicate the example given in docs…

can someone provide an example how to make it work in form tasks???

I can’t take screenshot in my machine it’s restricted but I’m using create form task activity and my goal is to create dynamic controls like checkbox and radio btn…

I have a dt col and based on the data option should populate…

I need to make it work for create form task in action center…

Can you give any sample for using dynamic checkbox and radio btn if possible?

I got result for dropdown already and I followed same steps but it’s not working for UiPath form task… For drop-down it works…
