Custom Messagebox

I want to display a custom message box with some labels and button. Anyone suggest how to do that?

Thanks for ur reply. I want two labels vertically like name occupation age etc. and two button sendmail and cancel

Hmm. i hope vertical label arrangement cant be done in Message box
