Click image in word file

I need to replace image in word file at least twice first replacement done correctly as I know the alt text but the second replacement… I can’t give the image alt text, the only way that I know to do that is by right click on image, but the click activity don’t work on image
Any one can help or give me a tip, thanks

For replacing the image from word file we have few activities like Replace image or Paste image into document.
