Accepting multiple inputs

Hi,
I want to accept multiple inputs from user. For e.g: FirstName,LastName,Age,DOB…

If I use Input Dialog, i have to accept one after the other.
Is there any better approach, to accept multiple inputs once, rather than accepting one at a time.
