Need help on OCR the Health Care Form HCFA1500 using OCR,

Problem Statment:
I need to extract data from the scanned copies of Health care form HCFA 1500 using OCR.
I need to extract the label i.e. header like First name i.e. Smith, Last name:John, etc.
And don’t want to show the top header like the company name i.e. xyx company name.(Attached screenshot for ref)

Try using ai center and train the document you should be able to extract…the document looks like a standard format so you can train on the forms ml and get the data
