Extracting Structured image from a PDF

I need to extract Structured image from a PDF. The PDF is not a readable PDF. it is image based PDF. So i need to use OCR. But i cannot preserve the structure of the PDF in the output of the OCR. Any help is highly appreciated. I have uploaded the sample image of how the tabular format looks like on a PDF
