Adding screenshot in word document for test cases

HI All, For my test case we need to take screenshot for each and every step and paste it in word document. Has anyone implemented this kind of approach? Also I was thinking after running the test cases can these word screenshot document can be attached to Test manager post execution in each and every test case?
