How do i read the data from PDF form?

Hi All

I need to extract the colored circle data from PDF form and i have attached PDF here.

I have used document understanding but i m not getting consistent run, L85.1 data returns empty values only but i need return “Yes”.

Can anyone know how do i extract only highlighted data?

Regarding second table(Encounter CPT codes) if highlighted 25, i need to extract 99214,25.
Regarding third table highlighted Q8 and 59 i need to extract 17110,59,Q8.

How to extract circled boolean fields in Document Understanding

Hi Rahul Thank you for your reply. I did apply a signature logic to the boolean field (assuming the circle was a signature) but the extracting confidence is low. The only way seems around it is through an ML extractor and UiPath has a documentation to it too. However, I only have one field to extract and wondering if there is any other way to extract other than deploying an ML skill and custom model.
