Windows form creation in uipath

i want to create windows form in uipath for taking inputs from user and store details in any format.
can anyone suggest me how to make such forms in uipath.

I want to do the form with
Inputs from user
submit button and
and after click it should store data.

I want to do that using packages.
Please any one suggest me how to do.
