条件分岐の設定もしくはアクティビティの配置の問題？

お世話になっております。
条件分岐の設定が間違っているのか、その後の流れが悪いのか想定しているように書き込まれないのでアドバイスいただきたいです。
添付のエクセル「一覧表」から「予定表」への入力で、予定表のエクセルの仕上がりイメージシートの②（赤枠）の部分がうまくいかず、条件分岐されずに書き込まれているのではないかと思っています。
現在作成しているものも添付していますので、間違っている箇所をご指摘いただけないでしょうか？
