Selecting data from DataTable based on 2 columns build data table activity

Hi;
I have a DT(sampleDTsampleDT.xls (31.5 KB)).I need to arrange and filter this DT using build dataTable activity(medcodesDT.xaml (10.0 KB)) for comparison to change medication names to medication codes and produce finalDT as shown in screenshot. (( Please see attached screenshots
