Read specific PDF information on scanned document with handwritten text, then transfer information from PDF to specific place on webpage

I am trying to extract text from a scanned pdf document that has human hand writing on it. I then want to transfer each specific number/information from the pdf to a webpage database that the information is entered. How do I do this? Any help is greatly appreciated!!
