Read checkbox in PDF

Hi Team,
I want to read the checkbox value friom PDF…can anyone suggest how it can done?
The pdf contains checkbox checked infront of No option and want to extract that.

Use google OCR for this.

i am currently using “Google Cloud Vision OCR” which extracts the text box perfectly. In order to use this, you need API key from google cloud account. So i have created a trial account and used that API key here. Below is the sample it extracted from the CMS 1500 claim form.
