Multiple Select Item - Can you create a prompt that has options?

I had a question regarding selecting multiple items.

Business Scenario:

We are looking to add forms from an document that is not accessible through either OCR or iFrame into an accessible web portal by typing in the name of the forms we are looking at from the non accessible OCR/iFrame window…

Is there a way to create a custom prompt that could contain all the options? There is no drop down menu with pre-existing values so selecting multiple items (activity) is not an option. I want to create CUSTOM select multiple items to use? How would I do this?
