How to send html page (survey page) via email with a couple of questions?

Hi,
I am quiet new to UiPath, and learning everyday. I have never worked with email activities before. So, I am stuck with a particular use case.
The requirement I have is that I need to send a html page via email with couple of questions in it.
The answers can be with radio buttons or checkbox in it. It should contain a submit button which will post the answers into a database. The purpose of this requirement is to automate a survey as much as possible.
If, anyone can help me in this matter it will be much appreciable.
