How to search for the documents based on check boxes

In the above form there are some checked and unchecked boxes available. If a person checked the box for Death Certificate, he needs to provide that file. Now we need to search for that check box is checked or not, if yes then seach the file with Person name is matching or not.

I have shared a sample Death Certificate here. we need to verify either it is Death certificate or not and either Name is matching or not.

Hello @raju_alakuntla, Can you please try with get attribute activity to test if it can identify if the box is checked or not? We can focus on the 2nd part after this test may be.
