How to map 5 column values into one column

Please find the below table

Code Currency English Maths Science Hindi Social
Asset INR H+2 H H+1 H+3 H+6
Check USD H+3 H+7 H+2 H+5 H+9
Asset INR H+2 H H+1 H+3 H+6
Ring USD H+3 H+7 H+2 H+5 H+9
Asset INR H+2 H H+1 H+3 H+6
Check USD H+3 H+7 H+2 H+5 H+9
Code Tplusvalue
Asset H+2
Check H+7
Asset H+2
Check H+1
Mid NA

I want to map all English, Maths, Science, Hindi, Social values which is only H+ values into Tplusvales column, if code is not found it should write NA

how do i achive this?
