"%localappdata%/Programs/task-capture/Task Capture.exe” --deactivate=“YourlicenseNo”

As per the docs, you need the Task Capture.exe to make it deactivate, That installer is a updater installer

Does this mean the only way to deactivate the license using ‘Request Form’?

Yes, also you can raise a request to technical team of UiPath too
