I recommend using a document scanner along with software such as Kodak Capture or Kofax Virtual ReScan (VRS), just to give some examples. Most products will offer automatic skew correction, either based on the scanner’s background (i.e. white paper on black background) or structural elements detected on a page (e.g. lines or text).

Ideally, this happens before the PDF is created as de-skewing pages later on may become more difficult.

As an alternative, Acrobat XI can correct skewed pages after they were scanned. Source.
