How BOT will validate which radio button enabled from the list and process further based on that?

I have a scenario like list of mail extensions (@ronaldjoshi.com…etc…6 are there). From the list of mail extensions BOT need to identify which mail extension is enabled then based on that BOT need to add that extension to the mail ID.

Instead of days and all, there will be extensions like shown in above image.

Can anyone help me on this issue.
