@damodar99 Why your Excel have 2 CoderId?

@damodar99 I have removed the first Coder Id column. Please find attached solution
Aetna- Weekly Performance-Template (2).xlsx (42.1 KB)
output.docx (63.3 KB) => I have changed the value of each field
Sequence14.xaml (11.8 KB) => Used Word activity Replace Text activity to replace with Excel data
