Generate new report in one single world doc

Dear guys

I try to extract data from Excel into the Word document. It extracts each row of the data in Excel to create one report, like this case;

Assume now I have 11 rows of data, how can I combine all these reports into one single Word document, but not create a Word document for every row of data in Excel?
All the report create in a same Word document like this;
