Drop Down List In Custom Input Dialog Box

Hello Everyone,

How can I create drop down list for the single Input field (Statement Type) in the following Image.
I have configured the Form using Custom Input Dialog Box.
Is there any way that we can change the (Statement Type) field to drop down list.
