Create multiple field input dialog

I don’t think so it possible as of now.
Only thing you can do is reduce number of activity in the workflow by making use of list and collection but same input dialog will appear every time.

Is it possible to choose multiple options in Input dialog?

I am able to create multiple options in the checkboxes as said in this answer Is it possible to design it in such a way that the user can select both the options ? Note : Jokes apart I am asking the technical perspective only
