Action Center Task Forms-File Attachment Exception

Hello Fellow UIPath Community.

We have gone live on a AI Machine document understanding initiative.

I have noticed an Action Center Exception for a file attachment in the Action Center Task Form

Exception Message: Classification result could not be fetch, Page index 1 cannot be found in DOM.

I downloaded the particular file attachment and added to a different queue item and the file attachment reflects in the Action Center Tasks Form.

The logs also do not indicate a particular exception for this file attachment…it is however in Action Center. Only 1-3 errors of this nature appear out of ± 1900 Action Center Tasks.

Could anyone have an idea on how this particular issue could be resolved, or what the issue could be??
