What do I use to capture user input like a box where user can describe the problem they are encountering. User can type in free form text.

Thank you all for your replies. although the input dialog box is not what I was expecting for an interface, but the simplicity makes it more favourable for my current requirement.
Sorry took a while to reply as was busy with other projects.
